Physicals and Concussion Baseline testing are in the same building. Appointments are required and will be scheduled adjacently or as close as possible. Please plan for 15 minutes for a Sport Physical or 30 minutes for both a Sports Physical and Concussion Baseline test. We will try to keep as close to a schedule as possible.

Middle and high school students must bring a completed Michigan High School Athletic Association Medical History/Consent form and Baseline Consent form signed by a parent or guardian. These forms are attached below. Parental attendance is encouraged. 

 

Physicals will be performed by our medical team of Sports Medicine Physicians, Orthopedic Sports Surgeons Physical Therapists, Athletic Trainers, and Central Michigan University Family Medicine Residents.

A concussion baseline assessment measures normal functioning in multiple areas of the brain that are commonly affected following a concussion. These allow for comparison of post-injury state to baseline parameters for accurate diagnosis and proper return to play protocol.

COVID-19 precautions and protocols will be followed to keep all attendees safe.
